To schedule an appointment with one of our offices, you will need to do the following:

  1. Obtain a prescription/referral from your physician. By state law, our staff can only perform rehabilitative care to a patient on the written prescription/referral of a licensed physician, osteopath or dentist. Currently, we may not, by law, accept a prescription/referral signed by a physician assistant.

    Any of these medical professionals may write a prescription for physical therapy just like a prescription for medication. This needs to be signed by one of these practitioners, dated, and include the medical diagnosis.

    Some insurance companies require their members to obtain a Primary Care Referral from their primary care physician, or obtain authorization from the insurance company prior to scheduling an appointment with us. Please check with the patient's insurance provider to see if such authorization is required.

  2. Select one of our locations that is most convenient to you. Locate one of our facilities from the enclosed Location Guide and contact the office directly.

  3. Contact the office of your choice to schedule the appointment. When you contact our office to schedule an appointment, our secretary will ask you a series of questions which will allow our staff to be prepared for your first visit. Please have available your prescription and/or primary care referral form or authorization number, your insurance card, and your social security number. The secretary will schedule your first appointment at that time and provide you with any further information you may need. Additional visits may be scheduled at the time of your first appointment.

  4. Print out and complete the forms listed on the right. The downloadable forms are available in Adobe .pdf format. If you do not have Acrobat Reader installed, download it for free by clicking here or the link on the right. Bring the completed forms with you to your first visit.

There are also some items that you will need to bring with you for your first appointment: your prescription and/or primary care referral form or authorization number, your insurance card, and your social security number. If the patient is a child under the age of 18 years, one of his/her parents will need to come to the first appointment to authorize care. Please wear comfortable, loose fitting clothing as appropriate.
